Ohio Licensed RN Care Manager

Job Summary

To support a growing practice, the full-time Ohio Licensed RN Care Manager will oversee the development and continuity of patient care plans, collaborate with field staff, and manage healthcare costs through effective care delivery and utilization management, all while working remotely.

Key responsibilities

• Review and revise the Plan of Care (POC) to ensure accuracy and compliance with regulatory standards
• Participate in interdisciplinary team meetings to facilitate collaboration and appropriate care planning
• Assess patient clinical, functional, and behavioral health status to ensure timely follow-up and service allocation

Required qualifications

• Registered Nurse licensure required
• Two years of home care field experience preferred
• One year of clinical review experience preferred
• Two years of case management and/or utilization management experience preferred
• Proficiency in HCHB preferred
