Ohio Licensed RN Care Manager

Job Summary

To support patient care continuity, the full-time Ohio Licensed RN Care Manager will oversee the development and implementation of patient care plans, collaborate with interdisciplinary teams, and manage healthcare costs through effective care delivery strategies in a fully remote setting.

Key responsibilities

• Review and revise Plans of Care to ensure accuracy and compliance with regulatory standards
• Participate in interdisciplinary team meetings to facilitate collaboration and appropriate care planning
• Assess patient clinical, functional, and behavioral health statuses to ensure timely follow-up and service allocation

Required qualifications

• Registered Nurse licensure required
• Two years of home care field experience preferred
• One year of clinical review experience preferred
• Two years of case management and/or utilization management experience preferred
• Proficiency in HCHB preferred
