Job Summary

A company is looking for a RN Care Navigator, responsible for case management specific to kidney health management.

Key Responsibilities

• Manage in and outbound calls while delivering exceptional service to members
• Educate members on kidney health, related co-morbid conditions, and optimize renal replacement therapy
• Coordinate care among patients, their support networks, and healthcare providers to ensure positive outcomes

Required Qualifications

• Active, unrestricted RN license required
• Bachelor's degree required
• CCM certification preferred
• Three years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
