Ohio Licensed RN Case Manager

Job Summary

Working remotely on a full-time temporary basis, the Ohio Licensed RN Case Manager will conduct comprehensive member assessments, evaluate healthcare needs, and develop care plans while ensuring compliance with regulatory standards.

Key responsibilities

• Conduct comprehensive member assessments using clinical tools and available healthcare information
• Evaluate member needs, eligibility, and benefit plan coverage while coordinating healthcare resources
• Develop and facilitate appropriate care plans and case management strategies to promote positive health outcomes

Required qualifications

• Active, unrestricted Registered Nurse (RN) license in Ohio or active Compact RN License
• Associate's Degree in Nursing (ADN) required; Bachelor's Degree in Nursing (BSN) preferred
• Minimum 3 years of clinical nursing experience in a hospital, home health, or ambulatory care setting
• Experience in conducting clinical assessments and evaluating patient needs
• Proficiency in navigating multiple computer systems and applications
