Job Summary
To support a growing healthcare initiative, the full-time RN Case Manager will conduct comprehensive member assessments, develop care plans, and coordinate services remotely while ensuring compliance with regulatory standards.
Key responsibilities
- Conduct comprehensive assessments of member healthcare needs and eligibility
- Develop and support care plans that promote optimal and cost-effective outcomes
- Coordinate services and advocate for members across healthcare programs and resources
Required qualifications
- Active unrestricted RN license in Ohio or Compact RN License
- Minimum 3 years of clinical nursing experience in relevant settings
- At least 2 years of case management experience
- Experience in the healthcare and/or managed care industry
- High School Diploma or equivalent
