Ohio Licensed Senior Care Specialist
Job Summary
To support members with chronic and complex health conditions, the seasonal, work-from-home Ohio Licensed Senior Care Specialist will conduct telephonic health assessments, develop individualized care plans, and coordinate services while utilizing clinical expertise.
Key responsibilities
- Conduct telephonic health assessments for Medicare, Medicaid, and group health plan members
- Develop actionable, individualized care plans based on member needs and health risks
- Collaborate with multidisciplinary teams to promote member self-management and improve health outcomes
Required qualifications
- Active, unrestricted Registered Nurse (RN) license in the state of residence
- Minimum of 2 years of clinical nursing experience; 3 or more years preferred
- Strong clinical judgment and critical-thinking skills
- Intermediate to advanced computer proficiency, including Microsoft Office applications
- Ability to work independently in a remote environment
