Job Summary

Seeking an Ohio Licensed Telemedicine Provider (NP or PA) for a full-time remote position focused on delivering comprehensive hormone optimization and preventive healthcare to patients throughout Ohio, with responsibilities including evaluating, treating, and managing patient care.

Key responsibilities:

• Evaluate and manage patients through individualized hormone optimization and medical weight management
• Provide preventive healthcare services aimed at improving overall health and wellness
• Maintain a balanced patient population with a focus on both men's and women's hormone replacement therapy

Required qualifications:

• Active Ohio Family Nurse Practitioner (FNP) license or Physician Assistant license
• Current DEA registration appropriate for practice in Ohio at the time of application
• Current National Provider Identifier (NPI) number without specialty
• Current national board certification applicable to the profession
• Experience or strong interest in hormone replacement therapy and preventive medicine is preferred
