Ohio Licensed Transitions Coordinator

Job Summary

A company is looking for a Transitions Coordinator-Post Acute.

Key Responsibilities

• Manage member level of care transitions to and from long-term care and HCBS waiver services
• Coordinate with interdisciplinary care teams to ensure compliance with regulatory requirements
• Assist members in maintaining benefit eligibility during transitions and support clinical documentation processes

Required Qualifications, Training, and Education

• Completion of an accredited RN degree program or degree required for Social Worker licensure in Ohio
• Preferred Bachelor of Science in Nursing (BSN) or equivalent degree in healthcare
• Minimum of three years of experience in case management or managed care
• Preferred experience in a Medicare and/or Medicaid managed care environment
• Preferred experience in post-acute, home care, waiver, or acute clinical care settings
