Ohio Licensed Transplant Nurse

Job Summary

To facilitate care coordination for members with potential transplant needs, the full-time Ohio Licensed Transplant Nurse will manage the transplant continuum from referral through post-transplant case closure, working onsite in Dublin, OH, with hybrid options for Columbus candidates and remote work available for those over 50 miles away.

Key responsibilities

• Identify and assess members for solid organ or bone marrow transplants and manage their care throughout the transplant process
• Document all member-related activities and maintain collaborative relationships with healthcare teams to advocate for members
• Educate members on the transplant process and provide health benefit information to support informed decision-making

Required qualifications

• Active, unrestricted Registered Nurse (RN) license in Ohio, with Multi-State License (MSL) preferred
• Bachelor's of Nursing degree preferred
• Minimum of two years of clinical experience with direct patient care
• Certified Case Manager (CCM) credential preferred or required within 24 months of hire
• Experience with solid organ or bone marrow transplants is desired
