Ohio State Licensed RN Care Manager

Job Summary

A company is looking for an Ohio State Licensed RN Care Manager for a 32-hour day shift position, to work remotely from home.

Key Responsibilities:

• Deliver targeted and patient-centered care in collaboration with patients, families, caregivers, and providers
• Promote optimal health through disease prevention and management strategies
• Apply evidence-based interventions for cost-efficient and outcome-based care

Required Qualifications:

• Bachelor's degree in nursing
• Current unencumbered RN or APP-RN licensure in the State of Ohio
• 2-5 years of recent nursing practice experience
• Knowledge of acute hospital, community, or public health nursing concepts and resources
• BLS - Basic Life Support certification issued by American Heart Association
