Job Summary

A company is looking for a Care Management Support Coordinator II.

Key Responsibilities
  • Perform outreach to members and support care plan next steps, resources, and scheduling
  • Connect members to health plan and community resources to ensure high-quality customer care
  • Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required Qualifications
  • High School diploma or GED
  • 1 - 2 years of related experience
  • Knowledge of assigned health plan activities and resources
  • Understanding of Social Determinants of Health (SDOH) needs
  • For Florida-Sunshine Health Plan, all interactions with members are done telephonically
