Job Summary

A company is looking for a Care Management Support Coordinator II.

Key Responsibilities
  • Perform outreach to members to support care plan next steps and connect them with resources
  • Serve as front-line support for member and provider inquiries, explaining care plan procedures
  • Document and maintain non-clinical member records in compliance with state and regulatory requirements
Required Qualifications
  • High School diploma or GED
  • 1 - 2 years of related experience
  • Knowledge of health plan activities and community resources
  • Ability to address Social Determinants of Health (SDOH) needs
  • Experience with administrative duties related to member onboarding
