Oklahoma Licensed Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and barriers to care, recommending plans for optimal outcomes
• Develop ongoing care plans and collaborate with providers to address unmet needs
• Coordinate between members, families, and care providers to ensure person-centered care

Required Qualifications

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
• 2 - 4 years of related experience
• RN - Registered Nurse - OK State Licensure and/or Compact State Licensure required
