Job Summary

A company is looking for a Care Manager (RN) to provide care management support for high-risk maternity members and newborns in the NICU.

Key Responsibilities

Evaluate member needs and recommend care plans to improve health outcomes
Coordinate care between members, families, and healthcare providers to ensure access to services
Monitor member progress and update care plans as necessary to meet goals

Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
2 - 4 years of related experience
Active RN license in Oklahoma
