Job Summary

A company is looking for an OK Licensed Clinician: LCSW in Behavioral Health within Family Medicine.

Key Responsibilities

• Provide customized care and treatment plans for patients in assigned Care Pod
• Coordinate behavioral health care with primary care providers and screen patients for disorders
• Monitor clinical symptoms, document progress, and facilitate care plan changes as needed

Required Qualifications, Training, and Education

• Master's Degree in Social Work, Counseling, or a related field
• Licensed to practice in Oklahoma as an independent clinician (LCSW/LICSW, LMHC, LMFT, LPC, or equivalent)
• Minimum of 1-5 years of clinical experience
• Fluent in medical interpretation in English and Spanish, both spoken and written
