Job Summary

To revolutionize healthcare delivery, the full-time Oklahoma Licensed Family Nurse Practitioner will conduct virtual wellness visits, perform health assessments, and collaborate with in-home clinical teams while working remotely.

Key responsibilities

• Conduct thorough health risk assessments and medication reviews during virtual wellness visits
• Collaborate with in-home team members to evaluate vital signs and arrange necessary patient care tasks
• Provide comprehensive care coordination, including preventive care interventions and referrals to specialists

Required qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner License in Oklahoma
• Experience performing wellness visits
• Technologically savvy and comfortable using digital tools for patient care
• Ability to work autonomously and maintain high-quality standards in patient care
