Job Summary

A company is looking for a LPN Care Navigator to manage patient care remotely.

Key Responsibilities

• Manage a panel of 400-450 patients, conducting monthly outreach and providing health coaching
• Collaborate with clinical teams to address patient needs and support care coordination goals
• Document all patient interactions in real-time while maintaining HIPAA compliance

Required Qualifications

• Active Licensed Practical Nurse or Licensed Vocational Nurse license with a multi-state compact license
• Must reside in the U.S
• Minimum 2 years' clinical experience
• Experience working with EMR systems and familiarity with chronic condition management
• Intermediate to advanced computer skills for navigating multiple systems
