Job Summary
To support post-acute communities and healthcare providers across Oklahoma, the part-time Weekend Oklahoma Licensed Post-Acute Liaison will conduct remote assessments, manage pre-admission screenings, and collaborate with facility Admissions Coordinators, working weekends with a preferred location in Oklahoma City or Tulsa metro area.
Key responsibilities
- Conduct pre-admission screenings and assessments to determine clinical, equipment, and transportation needs
- Develop and maintain strong referral development relationships with referral sources
- Collaborate with facility Admissions Coordinators to ensure a smooth, patient-centered admission experience
Required qualifications
- Current and unencumbered Oklahoma LPN or RN license
- Experience as a hospital liaison
- Experience in a skilled nursing or skilled rehabilitation setting
