Job Summary

A company is looking for an RN Care Coach who is passionate about helping Medicare patients achieve their health goals.

Key Responsibilities

• Provide non face-to-face care management services including health risk assessments and chronic care management
• Implement remote patient monitoring and transitional care management strategies
• Support patients in developing advanced care plans and preventive services

Required Qualifications, Training, and Education

• Current, unrestricted nursing license
• Ability to pass a background check and drug test
• Proficient computer skills, including experience with electronic health records (EHR)
• Self-motivated and goal-oriented with strong personal skills
• Availability to work 40 hours a week
