Oklahoma Licensed Telephonic Case Manager RN

Job Summary

A company is looking for a Telephonic Case Manager RN Medical or Oncology.

Key Responsibilities:

• Make outbound and receive inbound calls to assess members' current health status
• Identify gaps or barriers in treatment plans and provide patient education for self-management
• Coordinate care and make referrals to outside resources to ensure a complete continuum of quality care

Required Qualifications:

• Must reside in OK and hold a current, unrestricted RN license in the state of OK
• Active Compact RN License or willingness to obtain within 90 days of employment
• 3+ years of experience in a hospital, acute care, or direct care setting
• Ability to navigate a Windows-based environment and type efficiently
• Access to high-speed internet and a dedicated work area that ensures information privacy
