Job Summary

A company is looking for a Transition Specialist to support their Transitions of Care team in a remote role.

Key Responsibilities

• Support care coordination and develop effective transition plans for members
• Assist with member transitions based on enrollment or care service needs
• Educate members and families about services and coordinate care with providers

Required Qualifications

• Bachelor's degree required
• 2 - 4 years of related experience preferred
• Equivalent experience may be considered
• Knowledge of healthcare services and transition processes is beneficial
• Experience in care coordination or management is advantageous
