Oncology Advanced Practice Provider

Job Summary

To support cancer survivors through their journey, the full-time Oncology Survivorship Advanced Practice Provider will conduct comprehensive assessments, develop personalized treatment plans, and coordinate multidisciplinary care in a remote setting.

Key responsibilities

• Conduct thorough assessments of cancer survivors via telemedicine to identify their physical, emotional, and psychosocial needs
• Collaborate with the survivorship care team to create and update individualized survivorship care plans
• Provide education and support to patients regarding cancer survivorship and available resources

Required qualifications

• 3-5+ years of advanced practice experience in an oncology healthcare setting
• Master's degree in a relevant field
• Current certification as a Nurse Practitioner or Physician Assistant in an appropriate specialty
• Willingness to obtain licensure as an APP in specified states as required
• Experience in providing cancer survivorship care or strong symptom/supportive care management experience
