Oncology Care Nurse

Job Summary

A company is looking for an Oncology Transitions of Care Nurse.

Key Responsibilities

• Conduct clinical assessments and monitor changes in health for cancer patients
• Coordinate care transitions and educate members and caregivers about their diagnosis and treatment
• Assist with care coordination, including discharge plans and follow-up care with providers

Required Qualifications

• Bachelor of Science Degree in Nursing and a compact unrestricted Registered Nurse (RN) license
• At least 5 years of nursing experience, with 3 years in oncology nursing or case management
• Certification as a Certified Case Manager (CCM) is required
• Ability to work in a fast-paced environment and adapt to rapid changes
• Fluency in Spanish, both written and verbal, is preferred
