Oncology Care Transitions Nurse

Job Summary

To support members diagnosed with cancer, the full-time remote Oncology Care Transitions Nurse will conduct clinical assessments, coordinate care transitions, and educate members and caregivers about their treatment journey.

Key responsibilities

• Conduct telephonic clinical assessments and monitor changes in member health
• Coordinate care plans with hospital case managers and follow-up care with providers
• Build trusting relationships with members and assist with care coordination and management

Required qualifications

• Active, unrestricted compact Registered Nurse (RN) license with a willingness to obtain additional state licenses
• Degree in Nursing (ADN or BSN) required; BSN preferred
• At least 5 years of nursing experience, including 3 years in oncology nursing or case management
• Certified Case Manager (CCM) certification required
• Fluency in Spanish, both written and verbal, is preferred
