Job Summary
To support members diagnosed with cancer, the full-time remote Oncology Care Transitions Nurse will conduct clinical assessments, coordinate care transitions, and educate members and caregivers about their treatment journey.
Key responsibilities
- Conduct telephonic clinical assessments and monitor changes in member health
- Coordinate care plans with hospital case managers and follow-up care with providers
- Build trusting relationships with members and assist with care coordination and management
Required qualifications
- Active, unrestricted compact Registered Nurse (RN) license with a willingness to obtain additional state licenses
- Degree in Nursing (ADN or BSN) required; BSN preferred
- At least 5 years of nursing experience, including 3 years in oncology nursing or case management
- Certified Case Manager (CCM) certification required
- Fluency in Spanish, both written and verbal, is preferred
