Oncology Case Manager Nurse

Job Summary

To support members with complex or chronic conditions, the full-time Oncology Case Manager Nurse will provide proactive telephonic case management, develop individualized care plans, and advocate for appropriate services while working remotely from Arizona, Idaho, or Iowa.

Key responsibilities

• Identify and assess members who would benefit from case management using claims data and clinical records
• Develop and coordinate individualized care plans, facilitating communication among patients, providers, and health plans
• Provide telephonic case management for high-utilization patients, advocating for timely and cost-effective care

Required qualifications

• Active, unrestricted Registered Nurse (RN) license in good standing
• Associate's or Bachelor's degree in Nursing
• 5+ years of clinical nursing experience, with a focus on oncology nursing
• Experience in case management and managing complex, high-utilization patient populations
• Strong knowledge of chronic disease management and utilization management processes
