Oncology Clinical Pharmacist
Job Summary
A company is looking for an Oncology Clinical Pharmacist - Management Drug Therapy.
Key Responsibilities
- Evaluates and manages drug therapy for patients with specialty conditions
- Monitors treatment efficacy, safety parameters, and adherence for patients on specialty medications
- Collaborates with healthcare providers and documents therapy changes in the electronic health record
Required Qualifications, Training, and Education
- Bachelor of Science in Pharmacy or Doctorate of Pharmacy (PharmD) from an accredited College of Pharmacy
- 3 years of relevant experience or completion of an ASHP-accredited PGY-1 residency program
- Licensed Registered Pharmacist upon hire
- Completion of a pharmacy program from an accredited school of pharmacy upon hire
