Oncology Transitions of Care Nurse

Job Summary

To support patients navigating their cancer journey, the full-time Oncology Transitions of Care Nurse will conduct clinical assessments, coordinate care transitions, and educate members and caregivers over the phone in a remote setting.

Key responsibilities

• Conduct telephonic assessments and monitor changes in members' health while coordinating care and discharge plans with healthcare providers
• Build trusting relationships with members and their support networks, ensuring culturally competent care and addressing social determinants of health
• Collaborate with clinical leadership to communicate member feedback and assist with administrative projects as needed

Required qualifications

• Active, unrestricted compact Registered Nurse (RN) license and willingness to obtain additional state licenses
• Degree in Nursing (ADN or BSN) required; BSN preferred
• At least 5 years of nursing experience with 3 years in oncology nursing or case management, and Certified Case Manager (CCM) certification
• Experience with electronic health records and comfort in learning new technologies
• Fluency in Spanish, both written and verbal, is preferred
