Oncology Transitions of Care Nurse

Job Summary

A company is looking for an Oncology Transitions of Care Nurse.

Key Responsibilities:

• Conduct clinical assessments, monitor health changes, coordinate care, and educate members and caregivers over the phone
• Provide feedback from members and providers to clinical leadership to improve service offerings
• Collaborate with healthcare team, coordinate discharge plans, and establish support plans for members

Required Qualifications:

• Bachelors of Science Degree in Nursing and an unrestricted Registered Nurse (RN) license
• At least 5 years of nursing experience with 3 years of oncology nursing or case management experience
• Certification as an Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM)
• Comfort with using new technologies and learning new technology
• Ability to identify and prioritize member's needs and take immediate action
