Job Summary

A company is looking for a Care Coordinator - Determination of Needs.

Key Responsibilities

• Educate families about the Ontario Autism Program and available services
• Maintain accurate applicant information in the database
• Conduct determination of needs interviews to assess support requirements for children

Required Qualifications, Training, and Education

• Bachelor's Degree with a minimum of 3 years of experience in human services or a related field
• Experience working with families of children with Autism Spectrum Disorder or developmental disabilities
• Knowledge of the developmental services sector and community resources
• Fluency in French, Mandarin, or Cantonese is preferred
• Must have an up-to-date police reference check
