Job Summary

To ensure quality care for patients, the full-time Oregon and Washington Licensed Care Coordinator will manage patient assessments, coordinate care transitions, and collaborate with multidisciplinary teams while working remotely.

Key responsibilities:
  • Coordinate patient care by collaborating with clinical teams to review and improve treatment plans and manage a moderate caseload of patients
  • Ensure compliance with policies and regulations while documenting treatment plan progress and facilitating care transitions
  • Compile and report patient data, conducting quality management studies and audits to enhance care coordination
Required qualifications:
  • Minimum two (2) years of experience in behavioral health case management or care coordination
  • Master's degree in Psychology, Counseling, Social Work, or a related field with at least three (3) years of relevant experience
  • Licensure as a Clinical Social Worker in Oregon and Washington or equivalent licenses within six months of hire
  • National Provider Identifier required at hire
  • Experience in an inpatient psychiatric or psychiatric emergency service setting is preferred
