Job Summary
To ensure quality care in a remote setting, the full-time Oregon and Washington Licensed Care Coordinator will manage patient assessments, coordinate care transitions, document treatment plans, and collaborate with multidisciplinary teams to enhance patient outcomes.
Key responsibilities:
- Coordinate patient care by collaborating with clinical teams to review and improve treatment plans and facilitate care transitions
- Manage patient data and records, compiling and reporting information to support quality management studies and audits
- Ensure compliance with health care policies and regulations while advocating for patients during care transitions
Required qualifications:
- Minimum two (2) years of experience in behavioral health case management or care coordination
- Master's degree in Psychology, Counseling, Social Work, or a related field with a minimum of three (3) years of relevant experience
- Licenses required: Licensed Clinical Social Worker (Oregon) and Licensed Independent Clinical Social Worker (Washington) within 6 months of hire, or equivalent licenses
- National Provider Identifier required at hire
- Experience in an inpatient psychiatric or psychiatric emergency service setting is preferred
