Job Summary
To support a growing behavioral health team, the full-time Oregon and Washington Licensed Care Coordinator will ensure quality patient care by reviewing assessments and treatment plans, coordinating care transitions, and collaborating with multidisciplinary teams in a remote work environment.
Key responsibilities:
- Coordinate patient care by collaborating with clinical teams to review and improve treatment plans and facilitate care transitions
- Manage patient data and records by compiling information, conducting quality management studies, and documenting treatment plans
- Ensure compliance with policies and regulations while building relationships with external stakeholders to enhance care coordination
Required qualifications:
- Minimum two (2) years of experience in behavioral health case management or care coordination
- Master's degree in Psychology, Counseling, Social Work, or a related field with a minimum of three (3) years of relevant experience
- Licenses required within 6 months of hire: Licensed Clinical Social Worker (Oregon) and Licensed Independent Clinical Social Worker (Washington) or equivalent licenses
- National Provider Identifier required at hire
- Experience in an inpatient psychiatric or psychiatric emergency service setting is preferred
