Job Summary

To ensure quality care and support patient transitions, the full-time Oregon and Washington Licensed Care Coordinator will manage patient assessments, coordinate care needs, and collaborate with multidisciplinary teams remotely.

Key responsibilities:
  • Ensures comprehensive patient assessments and coordinates care transitions, including planning discharges and making community service referrals
  • Collaborates with clinical teams to review and improve treatment plans while documenting treatment progress and facilitating updates in clinical meetings
  • Manages patient data and records, conducting quality management studies and audits to support care coordination efforts
Required qualifications:
  • Minimum two (2) years of experience in behavioral health case management or care coordination
  • Master's degree in Psychology, Counseling, Social Work, or a related field with a minimum of three (3) years of experience in a directly related field
  • Licenses required: Licensed Clinical Social Worker (Oregon) and Licensed Independent Clinical Social Worker (Washington) within 6 months of hire, or equivalent licenses
  • National Provider Identifier required at hire
  • Credentials from the primary work state are required at hire; additional credentials from secondary states required post-hire
