Job Summary

To ensure quality care for patients, the full-time Oregon and Washington Licensed Care Coordinator will manage patient assessments and treatment plans, coordinate care transitions, and collaborate with multidisciplinary teams while working remotely.

Key responsibilities:
  • Coordinate patient care by collaborating with clinical teams to review and improve treatment plans and facilitate care transitions
  • Document treatment plan progress and participate in multidisciplinary clinical meetings to ensure comprehensive care
  • Manage patient data and records, compiling information for quality management studies and audits
Required qualifications:
  • Minimum two (2) years of experience in behavioral health case management or care coordination
  • Master's degree in Psychology, Counseling, Social Work, or a related field with at least three (3) years of experience in counseling or social work
  • Licensure as a Clinical Social Worker in Oregon and Washington, or equivalent licenses, required within six months of hire
  • National Provider Identifier required at hire
