Job Summary

To ensure quality care, the full-time Oregon and Washington Licensed Care Coordinator will manage patient assessments, coordinate care transitions, and document treatment plan progress while working remotely.

Key responsibilities:
  • Coordinate patient care by collaborating with clinical teams and managing treatment plans for a moderate caseload
  • Ensure compliance with policies and procedures, advocating for patients during care transitions and documenting treatment progress
  • Compile and report patient data, conducting quality management studies and audits to identify opportunities for care improvement
Required qualifications:
  • Minimum two years of experience in behavioral health case management or care coordination
  • Master's degree in Psychology, Counseling, Social Work, or a related field with three years of experience in counseling or social work
  • Licensure as a Clinical Social Worker or Mental Health Counselor in Oregon and Washington within six months of hire
  • National Provider Identifier required at hire
  • Experience in inpatient psychiatric or psychiatric emergency service settings is preferred
