Oregon and Washington Licensed Care Coordinator

Job Summary

To enhance patient care coordination, the full-time Oregon and Washington Licensed Care Coordinator will manage a moderate caseload by reviewing treatment plans, collaborating with clinical teams, and ensuring compliance with care standards while working remotely.

Key responsibilities:

• Coordinates patient care by collaborating with clinicians to review and improve treatment plans and facilitating care transitions
• Ensures quality of care through comprehensive patient assessments, documentation of treatment progress, and participation in multidisciplinary clinical meetings
• Manages patient data and records, compiling reports on services provided and conducting quality management audits

Required qualifications:

• Minimum two (2) years of experience in behavioral health case management or care coordination
• Master's degree in Psychology, Counseling, Social Work, or a related field with a minimum of three (3) years of experience in a directly related field
• Licenses required within 6 months of hire: Oregon and Washington clinical licenses (various options available)
• National Provider Identifier required at hire
