Job Summary

To ensure quality care in behavioral health, the full-time Oregon and Washington Licensed Care Coordinator will manage patient assessments, coordinate care transitions, and collaborate with clinical teams while working remotely.

Key responsibilities:
  • Review and document patient assessments and treatment plans, ensuring accuracy and completeness
  • Coordinate patient care by collaborating with clinicians, making referrals, and facilitating care transitions
  • Compile and report patient data, conducting quality management studies and audits to enhance care coordination
Required qualifications:
  • Minimum two (2) years of experience in behavioral health case management or care coordination
  • Master's degree in Psychology, Counseling, Social Work, or a related field with three (3) years of relevant experience
  • Licensed Clinical Social Worker (Oregon) and Licensed Independent Clinical Social Worker (Washington) within 6 months of hire, or equivalent licenses
  • National Provider Identifier required at hire
  • Experience in an inpatient psychiatric or psychiatric emergency service setting is preferred
