Oregon and Washington Licensed Care Coordinator

Job Summary

To ensure quality care in a remote setting, the full-time Oregon and Washington Licensed Care Coordinator will review patient assessments, coordinate care transitions, document treatment plans, and collaborate with multidisciplinary teams to enhance patient experiences across the continuum of care.

Key responsibilities:

• Coordinate patient care by collaborating with clinical teams to review and improve treatment plans and manage a moderate caseload of patients
• Ensure compliance with policies and regulations while advocating for patients during care transitions and documenting treatment plan progress
• Compile and report patient data to facilitate interventions and conduct quality management studies and audits

Required qualifications:

• Minimum two (2) years of experience in behavioral health case management or care coordination
• Master's degree in Psychology, Counseling, Social Work, or a related field and three (3) years of experience in counseling, social work, or a related field
• Licenses from Oregon and Washington required within six months of hire, including options for Clinical Social Worker, Mental Health Counselor, Marriage and Family Therapist, or Psychologist
• National Provider Identifier required at hire
