Job Summary

To ensure quality care for patients, the full-time Oregon and Washington Licensed Care Coordinator will manage patient assessments, coordinate care transitions, and document treatment plans while working remotely.

Key responsibilities:
  • Collaborates with clinical teams to review and improve treatment plans and coordinates care across the continuum
  • Documents patient progress and participates in multidisciplinary clinical meetings to update care strategies
  • Builds relationships with external providers and stakeholders to facilitate patient care and ensure compliance with regulations
Required qualifications:
  • Minimum two (2) years of experience in behavioral health case management or care coordination
  • Master's degree in Psychology, Counseling, Social Work, or a related field with at least three (3) years of relevant experience
  • Licenses required: Licensed Clinical Social Worker (Oregon) and Licensed Independent Clinical Social Worker (Washington) within 6 months of hire, or equivalent licenses
  • National Provider Identifier required at hire
