Job Summary
To ensure quality care for patients, the full-time Oregon and Washington Licensed Care Coordinator will manage patient assessments and care interventions, coordinate care transitions, and collaborate with clinical teams while working remotely.
Key responsibilities:
- Coordinate patient care by collaborating with clinicians, reviewing treatment plans, and making referral recommendations
- Ensure compliance with policies and regulations while documenting treatment plans and facilitating care transitions
- Manage patient data and records, conducting quality management studies and reporting on care metrics
Required qualifications:
- Minimum two (2) years of experience in behavioral health case management or care coordination
- Master's degree in Psychology, Counseling, Social Work, or a related field with three (3) years of relevant experience
- Licenses required from Oregon and Washington within six months of hire
- National Provider Identifier required at hire
