Job Summary

To ensure quality care for patients, the full-time Oregon and Washington Licensed Care Coordinator will manage patient assessments and care transitions, document treatment plan progress, and collaborate with multidisciplinary teams while working remotely.

Key Responsibilities
  • Coordinate patient care by collaborating with clinicians to review and improve treatment plans and manage a moderate caseload
  • Ensure compliance with policies and regulations while advocating for patients during care transitions and documenting treatment plan progress
  • Manage patient data and records by compiling reports and conducting quality management studies to identify opportunities for improvement
Required Qualifications
  • Minimum two (2) years of experience in behavioral health case management or care coordination
  • Master's degree in Psychology, Counseling, Social Work, or a related field with a minimum of three (3) years of experience in counseling or social work
  • Licenses required from both Oregon and Washington within six months of hire, including options for LCSW, LPC, LMFT, or Psychologist licenses
  • National Provider Identifier required at hire
