Job Summary

To enhance patient care coordination, the full-time Oregon and Washington Licensed Care Coordinator will manage a moderate caseload, ensuring quality treatment plans, collaborating with clinical teams, and documenting patient progress while working remotely.

Key responsibilities:
  • Coordinate patient care by collaborating with clinical teams to review and improve treatment plans and facilitate care transitions
  • Ensure compliance with policies and regulations while documenting treatment progress and conducting standard chart reviews
  • Manage patient data by compiling and reporting information, conducting quality management studies, and identifying opportunities for intervention
Required qualifications:
  • Minimum two (2) years of experience in behavioral health case management or care coordination
  • Master's degree in Psychology, Counseling, Social Work, or a related field with a minimum of three (3) years of experience in counseling or social work
  • Licensed Clinical Social Worker (Oregon) and Licensed Independent Clinical Social Worker (Washington) required within 6 months of hire, or equivalent licenses
  • National Provider Identifier required at hire
  • Experience in an inpatient psychiatric or psychiatric emergency service setting is preferred
