Job Summary

To ensure quality care and support patient transitions, the full-time Oregon and Washington Licensed Care Coordinator will manage patient assessments, coordinate care needs, and collaborate with clinical teams while working remotely.

Key responsibilities:
  • Ensure comprehensive and accurate patient assessments and treatment plans, advocating for patients during care transitions
  • Collaborate with clinical teams to improve care coordination and implement recommendations for treatment plans
  • Manage patient data and records, conducting quality management studies and audits to enhance care outcomes
Required qualifications:
  • Minimum two (2) years of experience in behavioral health case management or care coordination
  • Master's degree in Psychology, Counseling, Social Work, or a related field with three (3) years of relevant experience
  • Licensed Clinical Social Worker (Oregon) and Licensed Independent Clinical Social Worker (Washington) required within 6 months of hire
  • National Provider Identifier required at hire
  • Additional state credentials required post-hire based on the primary work state
