Job Summary

To ensure quality care in a remote setting, the full-time Oregon and Washington Licensed Care Coordinator will manage patient assessments, coordinate care transitions, and collaborate with multidisciplinary teams to enhance treatment plans and ensure compliance with regulations.

Key responsibilities:
  • Coordinate patient care by collaborating with clinical teams to review and improve treatment plans and manage a moderate caseload
  • Ensure quality of care by documenting treatment plan progress and advocating for patients during care transitions
  • Manage patient data and records by compiling information, conducting quality management studies, and reporting on care metrics
Required qualifications:
  • Minimum two years of experience in behavioral health case management or care coordination
  • Master's degree in Psychology, Counseling, Social Work, or a related field with three years of relevant experience
  • Licensure as a Clinical Social Worker, Mental Health Counselor, Marriage and Family Therapist, or Psychologist in Oregon and Washington within six months of hire
  • National Provider Identifier required at hire
