Job Summary
To enhance patient care coordination, the full-time Oregon and Washington Licensed Care Coordinator will manage a moderate caseload by reviewing treatment plans, collaborating with clinical teams, and ensuring compliance with healthcare regulations while working remotely.
Key responsibilities:
- Coordinates patient care by collaborating with clinicians to review and improve treatment plans and making referral recommendations
- Ensures quality of care by documenting treatment plan progress and facilitating smooth care transitions for patients
- Manages patient data and records by compiling information and conducting quality management studies and audits
Required qualifications:
- Minimum two (2) years of experience in behavioral health case management or care coordination
- Master's degree in Psychology, Counseling, Social Work, or a related field with a minimum of three (3) years of experience in counseling or social work
- Licensure requirements include obtaining Oregon and Washington clinical licenses within six months of hire
- National Provider Identifier required at hire
