Job Summary
To ensure quality care in a remote setting, the full-time Oregon and Washington Licensed Care Coordinator will manage patient assessments, coordinate care transitions, and collaborate with multidisciplinary teams to enhance treatment plans and ensure compliance with regulations.
Key responsibilities:
- Coordinate patient care by collaborating with clinicians to review and enhance treatment plans and manage a moderate caseload
- Ensure quality of care by documenting treatment plan progress and advocating for patients during care transitions
- Compile and report patient data to identify opportunities for improvement and ensure compliance with health care regulations
Required qualifications:
- Minimum two (2) years of experience in behavioral health case management or care coordination
- Master's degree in Psychology, Counseling, Social Work, or a related field with three (3) years of experience in a directly related field
- Licensure as a Clinical Social Worker in Oregon and Washington, or equivalent licenses, required within 6 months of hire
- National Provider Identifier required at hire
