Job Summary

To ensure quality care in a remote setting, the full-time Oregon and Washington Licensed Care Coordinator will manage patient assessments, coordinate care transitions, and document treatment plan progress while collaborating with multidisciplinary teams.

Key responsibilities:
  • Coordinate care for a moderate caseload of patients by collaborating with clinical teams and implementing treatment plans
  • Ensure compliance with policies and regulations while advocating for patients during care transitions
  • Manage patient data and records, compiling information for quality management studies and audits
Required qualifications:
  • Minimum two (2) years of experience in behavioral health case management or care coordination
  • Master's degree in Psychology, Counseling, Social Work, or a related field with three (3) years of relevant experience
  • Oregon and Washington licensure as a Clinical Social Worker, Mental Health Counselor, or equivalent within six months of hire
  • National Provider Identifier required at hire
  • Experience in an inpatient psychiatric or psychiatric emergency service setting is preferred
