Job Summary
To enhance patient care coordination, the full-time Oregon and Washington Licensed Care Coordinator will manage a moderate caseload, ensuring comprehensive treatment plans, collaborating with clinical teams, and documenting patient progress while working remotely.
Key responsibilities
- Coordinates patient care by collaborating with clinical teams and implementing recommendations to improve treatment plans
- Ensures quality of care by reviewing patient assessments and advocating for patients during care transitions
- Manages patient data and records, compiling information for quality management studies and audits
Required qualifications
- Minimum two (2) years of experience in behavioral health case management or care coordination
- Master's degree in Psychology, Counseling, Social Work, or a related field with three (3) years of relevant experience
- Licensed Clinical Social Worker (Oregon) and Licensed Independent Clinical Social Worker (Washington) within 6 months of hire, or equivalent licenses
- National Provider Identifier required at hire
- Credentials from the primary work state required at hire; additional credentials from secondary states required post-hire
