Oregon and Washington Licensed Care Coordinator

Job Summary

To enhance patient care coordination, the full-time Oregon and Washington Licensed Care Coordinator will manage a moderate caseload by reviewing treatment plans, collaborating with clinical teams, and ensuring compliance with health care policies while working remotely.

Key responsibilities

• Coordinates patient care by collaborating with clinical teams to review and improve treatment plans and facilitate care transitions
• Ensures quality of care through comprehensive patient assessments, documentation of treatment progress, and participation in multidisciplinary clinical meetings
• Manages patient data and records, compiling information for quality management studies and ensuring compliance with relevant policies and regulations

Required qualifications

• Minimum two (2) years of experience in behavioral health case management or care coordination
• Master's degree in Psychology, Counseling, Social Work, or a related field with a minimum of three (3) years of experience in counseling or social work
• Licensed Clinical Social Worker (Oregon) and Licensed Independent Clinical Social Worker (Washington) within six months of hire, or equivalent licenses
• National Provider Identifier required at hire
• Credentials from both Oregon and Washington required, with primary state credentials at hire
