Job Summary

To ensure quality care and support patient transitions, the full-time Oregon and Washington Licensed Care Coordinator will manage patient assessments, coordinate care needs, and collaborate with clinical teams while working remotely.

Key responsibilities:
  • Ensure comprehensive and accurate patient assessments and treatment plans, advocating for patients during care transitions
  • Coordinate care for a moderate caseload of patients by collaborating with clinical teams and making referral recommendations
  • Compile and report patient data to identify opportunities for interventions and conduct quality management studies
Required qualifications:
  • Minimum two (2) years of experience in behavioral health case management or care coordination
  • Master's degree in Psychology, Counseling, Social Work, or a related field with a minimum of three (3) years of experience in counseling or social work
  • Licensed Clinical Social Worker (Oregon) and Licensed Independent Clinical Social Worker (Washington) required within 6 months of hire, or equivalent licenses
  • National Provider Identifier required at hire
  • Credentials from multiple states required, with primary state credentials at hire and secondary state credentials post-hire
